1095-B Tax Forms
Tax form 1095-B provides proof that an employee and their family had health insurance during the year.
Beginning with the 2019 tax year, the government no longer requires tax form 1095-B to be filed with individual tax returns. This continues to be the case for the 2021 tax year. Therefore, a 1095-B will be provided upon request only.
If an employee would like a copy of their 1095-B, they may submit a request as follows:
- Call MESSA Member Services at 800-336-0013
- Secure message via their MyMESSA account
- Mail a request to MESSA, Attn: Member Services, 1475 Kendale Blvd., P.O. Box 2560, East Lansing, MI 48826.
If you, as an employer, need to request a copy of an employee’s 1095-B, call MESSA Group Services at 888-888-4167, ext. 7922.
Note: Employers are still required to provide employees with tax form 1095-C.
